Case Study #1:
Revenue Cycle Recovery: From Denials to Consistent Cash Flow
A 5 provider specialty practice contacted 107 Success initially concerned that the in-house billing team was struggling with their workload, understanding of their billing software, and lack of follow up on denied claims and patient billing.
After our initial assessment, over the next 3 quarters, the monthly average cash flow improved significantly. By addressing coding issues and claims follow up, the quarterly revenue payments from insurance companies increased an average of 19.2%. This initial change resulted in a positive average cash flow difference for the practice of well over six digits per month. Over the following year, by improving on the practice’s claims efficiency and follow up, 107 Success was able to reduce the overall accounts receivable balance by 40% while the charge amounts and patient counts remained stable. This has resulted in increased revenue and cash flow on a monthly basis for the practice compared to the two years prior to contracting with 107 Success.

After spending some time in the practice watching and talking with the team, we found that there were several positives, but one major thing was missing. The team was so busy focusing on the struggle of the day that consistent performance was difficult. 107 Success developed a plan and training program that improved the areas of scheduling, patient collections, and claims submission so that the practice could focus more on being more consistent each day. This consistency and focus all but eliminated the need to jump from problem to problem and allowed the practice to thrive again.

Launching a New Practice With Confidence
All new practices start with both excitement and anxiety. There are many different agenda items that require the new owner’s attention. Not to mention that it feels like everyone is asking for money before the money starts coming in. Many of our clients have come to us either as start up practices just getting started, or within their first year of operation once they realize that they are struggling with both seeing patients and getting paid at the same time.
One example we can share is of a solo physician practice that started from the beginning. While the physician focused on the medical side of their practice like getting a space, having the supplies on hand, hiring and training support staff and training on the EHR; we were hard at work setting up their practice management system, their schedule, their fee schedule, and all the business-related items. We credentialed the new practice and provider as a group for future growth opportunities and were ready to see patients and get paid for it within 90 days.
With the strong work prior to opening, 107 Success was able to quickly turn around claims and generate positive cash flow in the first month of operations. Within 90 days, the Days in AR had been running at less than a 30 day turn around meaning the practice was getting paid in well under 30 days for the claim. Payments from some payers were coming in as quickly as 7-10 days and patient balances were coming in within a 1-2 week time frame after the insurance had adjudicated the claim. With the focus being on generating positive cash flow quickly, the practice was able to keep their AR and their debt low while growing the practice quickly in the first year.
Case Study #4:
Rescuing a Practice After a Bad Billing Vendor
A common situation that practices deal with is the feeling that something is not right or could be better, but not knowing what it is or what to do about it. In this situation 107 Success was able to help a practice recover from a downward trend. The practice was made up of one physician and two mid-level providers.
The owner/physician contacted 107 Success out of concern with decreased cash flow and lower reimbursement amounts. The physician said multiple times during that initial call that they were working harder than they ever had and were making less but they couldn’t pinpoint what the issue was. 107 Success offered them a practice review so they could better understand their situation and opportunities. 107 Success quickly uncovered that the current billing company was billing expired CPT codes and CPT codes with missing modifiers resulting in unnecessary denials. With the increased denials the billing company was not keeping up with the workload and communication with the practice. A trickle-down effect of the denials was that patients were not being billed for their responsible amounts. As a result of the review, 107 Success took over the billing and patient inquiries immediately. Over the initial 90 days, 107 was able to collect a significant amount of patient balances while coaching the providers on which CPT codes were valid and potential replacements for the expired codes that were being used. Over the next 6 months the practice experienced a 10% increase in monthly revenue from insurance carriers simply due to eliminating many of the denials. An additional consequence was that patients were being billed for their responsibility in a timelier manner resulting in a higher net collections percentage and lower write-offs of bad debt balances.
With 107 Success being able to handle the billing questions from patients directly, it freed up the office staff to focus on the patients in the building and improve the efficiency and accuracy of data collection resulting in a more efficient claims process. With improved communication, reduced denials, and increased patient payments the practice was able to sustain a consistent 10% growth rate year over year for the next 3 years.
